2 Eligibility & Accounts
Patient accounts are self-registered and require admin approval before full access. You must provide accurate information and a valid ID for verification. Each account is for a single authorized user; sharing credentials is prohibited.
3 Acceptable Use
Use the portal only for its lawful health-service purposes. Do not access records that are not yours, attempt to disrupt or probe the system, or upload false, malicious, or unlawful content.
4 Health Disclaimer
Records here are for reference and surveillance only and do not replace professional medical advice, diagnosis, or treatment. In an emergency, call 911, CHO II at (046) 416-5638, or the DOH hotline 1555.
5 Data Privacy (RA 10173)
Your personal and health information is protected under the Data Privacy Act of 2012 and used solely for healthcare delivery and dengue surveillance. Passwords are stored as one-way hashes, access is role-restricted, and your data is never sold to third parties. Uploaded IDs are used only for verification.
6 User Responsibilities
Keep your credentials confidential, log out on shared devices, and notify CHO II of any unauthorized use. You are responsible for activity under your account.
7 Liability & Changes
The portal is provided “as is.” CHO II is not liable for losses from use of, or inability to use, the service except as required by law. These Terms may be updated; continued use means acceptance of the revised Terms.
