How do I register?
You can register yourself online: on the sign-in page click "Register", fill in your details, choose a valid government-issued ID and upload clear photos of its front and back (a parent's ID may be used for minors), and choose your own password. The City Health Office reviews your ID and approves the account; you are told by email. You may also register in person at the City Health Office II in Brgy. Sta. Cruz I; the office then emails you a link to set your own password. Staff never see or set it.
Who can access this portal?
Six kinds of users, each seeing only what their work needs: Patients (their own record and appointments), Medical Technologists (encode lab results and handle appointments), Barangay Health Workers (file daily Search & Destroy), Barangay Councils (file BADAC reports), the City Health Office II administrator, and I.T. (accounts, security and backups). Everyone signs in on the same page; the system opens the right portal for the account.
How do I reset my password?
On the sign-in page, click "Forgot Password" and enter your registered email. A 6-digit code is sent to your email; it is valid for 15 minutes. Enter it, then set a new password. Changing your password signs you out on every other device. If no email arrives, check your spam folder or contact the health office.
How do I update my profile information?
After signing in, open "Account" and click Edit. You can change your name, email and mobile number, and change your password. Your birth date, sex and barangay are verified by the City Health Office, so contact the office to correct them.
Health Records & Lab Results
Where can I view my lab results?
Sign in and open "My Record". Your laboratory results (such as the dengue NS1, IgG and IgM test, CBC and platelet count) are listed there, newest first. Each result is also emailed to you as a PDF; to open it, type your birth date as DDMMYYYY.
How do I report a dengue case?
A dengue case is confirmed by a positive NS1 test at the City Health Office II laboratory, and the result is recorded in the portal by the medical technologist. If you have symptoms, call the City Health Office II at (046) 416-5638 or visit in person; patients do not file case reports themselves.
What are dengue alert levels?
The health office watches two signals per barangay: a barangay becomes a hotspot when its confirmed cases rise for two weeks in a row, which schedules fogging; and the Search & Destroy indices flag a high mosquito-larvae risk when the House Index is above 5% or the Breteau Index is above 20. The office can also mark a patient's own record with an alert level.
Is my health data kept private?
Yes, under the Data Privacy Act of 2012 (RA 10173). You see only your own record. Staff see only what their work needs: the medical technologists who encode your results, and the City Health Office administrators. Barangay staff never see patient records. Every change to a record is logged with who made it, and your ID photos are served only through a checked, signed-in link.

What is Search & Destroy?
Search & Destroy (S&D) is the daily community drive against dengue mosquitoes. Every day at 4:00 PM, each barangay's health worker leads a house-to-house check of every village to find and destroy breeding sites, such as water in containers, old tires, flower vases and drums, and files the result in the portal, house by house if they wish. The City Health Office validates each filing.
What is BADAC compliance?
BADAC is the Barangay Anti-Drug Abuse Council. Each barangay council files its documentary requirements in the portal, monthly, quarterly or yearly depending on the document, by the 10th of the month. The City Health Office reviews each report and approves it or returns it with a note; a late report is still accepted but marked late.
